Hello Dr. Loai, i am glad to find such a blog where i can ask medical questions and have answers..i liked the idea; it is interactive and useful..I don't know how much you can help, but my child has a chronic constipation and i have to give him a medicine called senna and once i stop it the constipation is back! my baby now passes loose stool all the time! my baby is 11 months old and i am concerned. pls advise.

I can tell you that laxative dependency (which may or may not be the cause of constipation in your baby) is a condition where patients receiving laxatives for constipation becomes dependent on the use of these medications. This means whenever they attempt to stop the use of the laxative constipation comes back. It can occur in elderly, adults, adolescents, children and even infants. It is not clear if this is really due to the laxative or if this is due to the underlying chronic condition that leads to chronic constipaion.
Some infants will have constipation if they do not receive the correct formula for their age, or if they do not receive more frequent and smaller meals. The risk from the chronic use of the laxatives is one of the baby becoming dehydrated and hence at risk for contraction of their blood volume which may have significant consequences which you certainly and correctly should seek advice from experts to make sure that you can avoid in your baby.
There multiple things you can do to solve this problem. First, if you know the cause of this constipation, seek specific advice with how to overcome this cause. For example, if your baby is not receiving the right formula, or if the feeding habits need to be altered then that may be all that you need to do to resolve this problem. Second, try to give more fluids and attempt tapering the laxative slowly over several days with small decreases in the dose every day or few days. Third, if the above does not work, you can try to use another laxative from a different class, increasing the new laxative dose slowly while tapering the laxative your baby is already on slowly. For example, you can try introducing lactulose at 2.5 ml twice daily and then try and taper the senokot (senna) slowly (e.g. give in a syringe 6.0 ml once instead of 8 for a couple days then 4.0 for couple more days, then 2.0 for couple more days and then off). One last thing is that you can try and use formulas with higher content of fiber while tapering the laxatives off.
